This is a generic expense form template. It is designed to handle up to 2 types of taxes and is initially configured for the Province of Ontario, Canada to use PST and GST. It is also multi-currency to handle expenses in U.S. Dollars as well as Canadian.

You can add your own categories by editing the "Categories" sheet and you can add/edit your own departments, positions, and managers.
